Waiver forms

For unaccompanied minor participants

If a participant is under 18 and will not be accompanied by their legal parent or guardian, they will need to bring a release form signed by their legal parent or guardian with them to the park.

These waivers must be printed and completed before arriving for check-in.

If a participant who is under 18 arrives for check-in without a release form signed by their legal parent or guardian, they will not be allowed on the course.

For Guardians of Unaccompanied Minors

For adult or accompanied minor participants

For participants who are 18+ or for minor participants (under 18) who will be accompanied by their legal parent or guardian during participation, waivers can either be printed and completed ahead of time to be brought to check-in (allowing for faster check-in time) or be picked up and completed at check-in.

For Adults and Accompanied Minors

  • Sugar Works/The Villages programs involve a variety of events, which at times could include walking over uneven terrain and walking or climbing on activities at height. Even with Sugar Works/The Villages’ strong emphasis on safety, by participating in the course, there is always an inherent risk which must be assumed by each participant.

    The level of participation in a Sugar Works/The Villages activity is at all times completely voluntary and up to the participant’s own individual choice. Each participant will have the opportunity to inquire about the Sugar Works/The Villages activities in which they plan to participate and may ask any questions regarding those activities and the risks involved.

    Participants may be photographed while participating in Sugar Works/The Villages events, and Sugar Works/The Villages may use such photographs for any lawful purpose, including publicity, advertising, and web content.
